What is OhioRISE?
OhioRISE supports children and youth with behavioral health needs while helping families feel heard, understood, and supported along the way. Through family-centered, community-based care, services are shaped around each child’s unique strengths, needs, and goals. Support may include respite care, where a trusted provider spends time with the child at home or in the community through safe, engaging activities while giving caregivers a much-needed temporary break.
At Sincere Hearts, we believe supporting the entire family helps create stability, stronger connections, and hope for a brighter future. Respite services are based on the child’s eligibility and approved care plan.

Our respite services provide children and youth with a safe, encouraging space to feel understood, build confidence, and enjoy meaningful experiences. Through personalized activities and caring guidance, we support emotional well-being, positive behavior, communication, and healthy coping skills.
We also create opportunities for children and youth to connect with others and participate in their communities through recreational activities, educational experiences, and age-appropriate outings—all based on their interests, needs, and approved care plan.

How to Get Started
Contact Ohio Medicaid
Begin by contacting your child’s Medicaid managed-care plan, your local OhioRISE Care Management Entity, or Aetna Better Health of Ohio.Complete a CANS Assessment
A certified assessor will meet with your family to learn about your child’s strengths, needs, experiences, and goals. This assessment helps determine OhioRISE eligibility.Connect With a Care Coordinator
If eligible, your family will be connected with an OhioRISE care coordinator who will help create a care plan and identify appropriate services.Choose Your Provider
Once services such as respite are included and approved in the care plan, your care coordinator can help your family connect with an approved provider.
Need help knowing where to begin? Sincere Hearts can provide general information, but Ohio Medicaid and OhioRISE determine eligibility and authorize services. To request a CANS assessment, contact your Medicaid plan, local Care Management Entity, Aetna OhioRISE, or the Ohio Medicaid Consumer Hotline at 1-800-324-8680 (TTY: 711).
